What We Do

• Care Coordination: We connect all the existing members of the patient’s medical care team, including their GP, social workers, and therapist, thus ensuring cohesive care, and easing the burden of communication for parents. This is critical during times of increased medical need, changes in treatment, and other transitions.
• 24/7 On-Call Service: Client families can reach us any time on our dedicated phone line. Every call will be answered by a registered manager who is familiar with the patient and can aid in urgent but non-emergency situations, providing prompt medical intervention when required.
• Identifying Gaps in Care: We take time to understand the child’s current medical challenges to better help us recognise gaps in care and unmet medical needs. With the support of the child’s family, medical team, and existing resources, we will develop a comprehensive care plan for your child.
• Emotional Health: Our support workers prioritise the emotional health needs of both patients and their family members. While we cannot change the inherent difficulties of living with chronic health challenges, we can cultivate the emotional resilience needed to live well with serious illness. We attend to the emotional effects of long-term illness for both patient and family by fostering resilience, improving coping strategies, reinforcing stress management skills, and increasing emotional intelligence.
• Social Activities: We support children to participate in social activities and encourage them to learn new skills and develop in a safe stimulating environment. Carers are responsible for making a list of behaviours, attitudes, strengths, and shortcomings to better plan activities to meet the child’s needs.
• Therapeutic Services: We liaise and collaborate with children, their families, and other healthcare professionals to provide therapeutic services where needed. What’s more, we work proactively with other agencies and professionals and seek to build effective working relationships with parents, teachers, social workers, and local authorities responsible for your child.
• Physical Support and Complex Care: All care must be tailored to your child’s needs. We support children who need assistance with basic everyday tasks such as personal care, toileting, feeding (Oral or PEG tube feeding), or minor chores in day-to-day life. We provide transfers for children who are fully wheelchair-dependent.
• Documentation: Our team will complete all required documentation and progress records, including care plans and incident reports. We will also take part in meetings about care and supervision and work alongside professionals such as doctors, nurses, teachers, social workers, and psychologists.
• Health and Safety: All completed risk assessments are communicated to our team. Our staff have received adequate training in manual handling and training in the use of specific equipment such as slings and hoists, lifts, motorised ramps, bed rails, and shower beds.
