What We Do

  • Care Coordination: We connect all the existing members of the patient’s medical care team, including their GP, social workers, and therapist, thus ensuring cohesive care, and easing the burden of communication for parents. This is critical during times of increased medical need, changes in treatment, and other transitions.
  • 24/7 On-Call Service: Client families can reach us any time on our dedicated phone line. Every call will be answered by a registered manager who is familiar with the patient and can aid in urgent but non-emergency situations, providing prompt medical intervention when required.
